Six ways this form comes back, and what the tool does about each

A rejected form costs a second trip. Every rule below is enforced or surfaced before you can export.

  • A credited attribute with no photo

    Questions 2 through 9 each have their own photo step. You cannot pass one without a photo, or an explicit acknowledgement that is recorded on the report.

  • The wrong “weakest” answer

    Questions 5, 6 and 9 ask for the weakest feature present, not the best. The instruction is the loudest thing on those screens, and question 9 is derived from the chart for you.

  • A transition-year home with no permit date

    A home built in 2002, 2003, 2007, 2008 or 1994–96 needs its permit application date. The field appears exactly when the year you entered requires it.

  • A roof marked hip that is not one

    Enter the non-hip length and the perimeter and the 10% test is computed live, telling you whether it qualifies before you commit to the answer.

  • Submitting the retired form

    Florida’s own site still serves the pre-2026 form at several URLs. This is built on the Rev. 04/26 document adopted under Rule 69O-170.0155, verified against the rulemaking record.

  • The quirks that are not on the form at all

    Citizens asking for roof-slope images. State Farm denying condo opening protection unless the whole building is protected. A county stamp refused without an NOA number. You are warned before you submit, not after.

How it works on site

Nothing leaves your device unless you ask

Reports, photos and the PDF are all produced in the browser. Sign in and you get a backup you can reach from another phone; stay signed out and your clients’ addresses never reach a server at all.

The form changed on 1 April 2026

Rev. 04/26 replaced a version that had been in use for over a decade. Photographs are now required for questions 2 through 9, not 3 through 7. Question 6 gained a set of minimal qualifying conditions. Most guides online still describe the old numbering.

This tool is built on the document adopted under Rule 69O-170.0155, F.A.C., and checks the rulemaking record for amendments rather than assuming none have been filed.
